Supportive Concepts for Families is seeking compassionate Certified Nursing Assistants (CNAs) to serve as Community CNAs, providing direct health support and personal care to adults with intellectual and developmental disabilities living in community homes throughout Pennsylvania.
This is a field-based position where you'll work alongside Community Health Nurses, Direct Support Professionals (DSPs), physicians, specialists, and interdisciplinary teams to promote health, independence, dignity, and quality of life for the individuals we support.

No hospital environment or routine bedside assignments
What You'll Do
Provide hands-on personal care and support with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility assistance.
Monitor and document vital signs, weight, hydration, nutrition, and overall health status.
Observe for changes in physical or behavioral condition and promptly report concerns to the Community Health Nurse.
Assist with routine health monitoring and wellness visits.
Support medication administration processes as permitted by state regulations and organizational policy.
Accompany individuals to medical appointments when needed and communicate observations to the healthcare team.
Assist with infection prevention, safety monitoring, fall prevention, and health promotion activities.
Encourage independence while respecting each individual's choices, preferences, and dignity.
Maintain accurate documentation in accordance with agency policies and regulatory requirements.
Collaborate with Direct Support Professionals, Community Health Nurses, physicians, and interdisciplinary teams to ensure coordinated care.
Qualifications
Active Pennsylvania Certified Nursing Assistant (CNA) certification in good standing.
Valid driver's license and reliable transportation.
Compassionate, dependable, and committed to person-centered care.
Strong communication, observation, and organizational skills.
Ability to work independently while collaborating with an interdisciplinary healthcare team.
Experience working with individuals with intellectual or developmental disabilities, long-term care, home health, behavioral health, or community-based services is preferred but not required.
