What are the responsibilities and job description for the High Fidelity Wraparound Facilitator position at Support Incorporated?
Support Incorporated is seeking a passionate and qualified professional to join our growing High Fidelity Wraparound (HFW) team. The High Fidelity Wraparound Facilitator plays a vital role in helping children, youth, and families achieve stability, empowerment, and long-term success through the wraparound process. This position ensures that services are provided with fidelity to the model and that the family’s voice and choice guide every decision.
Key Responsibilities
- Partner with youth and families to identify their strengths, needs, and priorities.
- Engage and coordinate a team of service providers, natural supports, and community partners to build an integrated, family-driven plan.
- Facilitate regular team meetings, ensuring progress toward goals and alignment with the family’s vision and mission.
- Maintain consistent communication with families to identify emerging needs and adjust the plan as necessary.
- Support families in developing self-advocacy and self-management skills to sustain progress independently.
- Strengthen natural supports and encourage active participation from family, friends, and community members—ensuring that at least half of the plan’s action steps are supported by natural resources.
- Monitor team satisfaction, maintain engagement, and ensure wraparound principles are upheld.
Qualifications
- Must meet the requirements of a Qualified Professional as defined by NC Medicaid.
- Bachelor’s degree in a human services–related field (experience may be considered in place of degree).
- Two (2) years of experience providing direct services to children and families.
- Must pass criminal background, child and adult abuse registry, and motor vehicle checks.
