Job Title: Admissions Nurse – Hospice / Palliative / Oncology

Compensation: $200 - $250 per admission helping with standard nursing visits ($40 - $50 per visit)

Schedule: Full-Time or Part-Time | Includes 1 on-call shift per week (backup) and every 4th–7th weekend

Position Summary:
We are seeking a skilled and compassionate Admissions Nurse (RN) to serve as the face of our hospice team across communities in the Greater Twin Cities. This is a road warrior position perfect for nurses with prior hospice, oncology, or palliative care experience, either directly in hospice or in acute-care settings with patients nearing end-of-life. This role is integral to our growing team, with room for personal and professional fulfillment.
Key Responsibilities:
- Travel to patient residences and senior communities to complete hospice admissions.
- Assess clinical eligibility and initiate services with warmth, clarity, and professionalism.
- Educate patients and families on hospice philosophy, benefits, and next steps.
- Collaborate with physicians, intake coordinators, and interdisciplinary teams.
- Document all clinical findings and care coordination accurately and promptly.
Qualifications:
- Active RN license in the State of Minnesota.
- Experience in hospice, oncology, or palliative care required (home care or hospital).
- Comfort discussing end-of-life care with patients and families.
- Strong clinical assessment and communication skills.
- Valid driver’s license and reliable transportation.
