Responsibilities
- The Social Services Assistant works with the MSW Case Manager in identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his/her stay to assist in the planning for his/her discharge.
- Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits.
- Follow up and resolve any patient grievances.
- Facilitate the social and psychological well-being of residents and their families. Conduct a brief interview of Mental Health Status and Geriatric Depression Scale.
- Plan discharges/transfers to ensure appropriateness and continuity of care for transfers within and discharges from facility. Set up transportation as necessary, arrange Home Health services, and attend weekly Medicare meetings.
- Involve the entire facility in meeting residents’ psychosocial needs and make recommendations to staff where identified.
- Work cooperatively with supervisors and staff and maintains a good working relationship with all staff. Establishes good working relationships with all facility staff.
- Provide customer service to ensure internal and external customers of the facility have a positive experience.
