Role Description
This is a PRN role for a Medical Social Worker located in Higland Village, TX. The Medical Social Worker will provide emotional and psychosocial support to patients and their families, facilitating discharge planning, coordinating case management, and collaborating with the interdisciplinary team. Daily responsibilities include conducting patient assessments, connecting patients and families with community or social services, and assisting in navigating complex healthcare systems to ensure a continuum of care.
Qualifications
- Experience in Medical Social Work and Social Work
- Proficiency in Discharge Planning and Case Management
- Knowledge of Social Services and the ability to connect patients and families with community resources
- Strong interpersonal and communication skills for collaboration with interdisciplinary teams
- Master's degree in Social Work (MSW) from an accredited program
- Licensure as a Social Worker in the state of Texas
- Experience in hospice or palliative care is a plus
- Proven ability to work empathetically with diverse populations and manage complex family dynamics
