We are seeking a compassionate and dedicated Medical Social Worker specializing in hospice care to join our team. Duties
- Conduct comprehensive patient assessments to evaluate psychosocial needs, mental health status, and family dynamics.
- Develop individualized care plans incorporating behavioral health strategies, crisis intervention techniques, and counseling approaches such as psychotherapy and motivational interviewing.
- Provide individual, family, and group therapy sessions focused on grief support, behavioral health challenges, addiction counseling, or dementia care.
- Coordinate with interdisciplinary teams to facilitate seamless hospice care delivery, including intake procedures and medical documentation in compliance with HIPAA regulations.
- Manage case documentation accurately within electronic medical records (EMR) systems, ensuring all medical records and clinical notes reflect current patient status and treatment plans.
- Offer crisis management support during urgent situations related to mental health or behavioral health crises.
- Assist with data collection for clinical research projects related to hospice care outcomes or behavioral health interventions.
Qualifications
- Bachelor's or Master’s degree in Social Work (MSW preferred), with current licensure or certification as applicable by state law.
- Proven experience in hospice care, hospital settings, outpatient clinics, or home & community care environments.
- Strong knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and medical documentation standards.
- Demonstrated expertise in child & family counseling, dementia care (including Alzheimer’s), PTSD care, working with individuals with disabilities or developmental disabilities, and working with children or geriatrics populations.
- Proficiency in EMR systems and data collection tools; familiarity with HIPAA regulations is essential.
- Excellent communication skills for intake interviews, diagnostic evaluation, case management, and behavioral therapy sessions.
- Ability to handle crisis situations effectively while maintaining professionalism and empathy.
- Experience working with diverse populations including assisted living residents, inpatient settings, nursing homes, or outpatient clinics is highly desirable.
