Schedule: Day Shift | 3x12s (7a–7p) | Every other weekend rotation

- Coordinate Clinical Care: Work closely with interdisciplinary teams and external providers to ensure seamless updates on clinical information.
- Drive Treatment Planning: Lead the admission process by completing thorough psychosocial assessments and helping build a roadmap for recovery.
- Connect the Community: Provide vital resource information and "warm-referrals" to community providers to ensure ongoing engagement.
- Manage Admissions: Handle the intake process with excellence, including verification of benefits and necessary clinical authorizations.
What We're Looking For:
- Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.
- Must be 21 yrs or older.
- License: BSW, LSW, LISW, LPC, LPCC, MFT
- Mental Health Therapist
- Social Services
- Licensed Professional Counselor
