What are the responsibilities and job description for the Behavioral Health Professional Clinician - Geriatric Transitions Program position at SUNRISE SERVICES INC?
Duties & Responsibilities:
- Assess client need, risk factors and barriers to community placement at referral
- Identify existing services and supports (informal and formal) involved with client at time of referral. Seek to identify most appropriate role and input to compliment and extend existing service provision in the interest of facilitating a successful community placement.
- Coordinate and facilitate care conference meetings to problem solve and address issues relating to challenging behaviors and community placement
- Create effective client specific positive behavior support plans / Individual support plans to be utilized by clients and residential providers in community settings. Monitor use of the plans and provide trainings and consultation as needed to ensure effective use of the plan. Update and modify plans as needed.
- Provide crisis intervention and consultation by phone as needed.
Respond to out of hours requests for crisis intervention and consultation as needed, on a rotational basis.
Coordinate services with community agencies such as DSHS, doctors, hospital staff, discharge planners, other MH Providers, courts, Day Services, Home Care agencies, Employment Support agencies, residential providers etc. - Record, track and monitor presentation of challenging behaviors at referral, placement, 30, 60 and 90 days as needed
Provide services and treatment in accordance with the evidence-based practice recovery model, and document the same.
In coordination with community agencies and professionals, provide mental health services as appropriate including: direct clinical services such as intake, assessment, diagnosis, treatment, and crisis intervention; consultation/liaison with government and community agencies, hospitals, and other programs; community education, development, coordination and prevention/intervention programs; and program development and evaluation.
Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

Education: Master’s degree or further advanced degree in counseling or one of the social sciences OR psychiatric nurse.

Experience: At least two years of experience in supervised direct treatment of persons with mental illness and/or developmental disabilities, or a Mental Health Professional (MHP), or a professional with significant career related experience working towards hours of completion for MHP status.

Licensure/Certification: MHP (LICSW, LMHC, LMFT, associates level clinicians are welcome to apply) or Agency Affiliated Counselor License.

DSHS background clearance required.

Current valid Washington State Driver’s License.

Skill using Microsoft Office products including Word and Outlook.

Strong verbal and written communication skills, and demonstrated leadership ability.

Flexible. Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral and staffing issues.

Skill in reading, synthesizing, and acting upon multifaceted information.
- Extensive knowledge of protocols of NSMHA.
Compensation:
- $32.36 - $36.52/hr
- All wages depend on applicable experience, license, and education.
Schedule:
Monday - Friday, Full-time ideal, open to part-time
