Job Summary
This is a challenging and rewarding opportunity for an experienced Nurse to join our team as a Medication Care Manager. The successful candidate will be responsible for providing high-quality care and services to our residents.
Key Responsibilities
- Administer medications and treatments as prescribed by physicians
- Maintain accurate and up-to-date medication records
- Conduct regular Service and Health Updates (SHUs) to ensure resident well-being
- Participate in the development of Individualized Service Plans (ISPs) to meet resident needs
- Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure seamless care delivery
Requirements
- LPN, LVN, or state/province specific licensed nurse credential
- Must maintain certifications and ongoing training as required by Sunrise and state/provincial regulations
- Previous experience working with seniors preferred
- Ability to make choices and decisions in the best interest of residents
- Excellent communication and problem-solving skills
