Position Overview
Sunrise Recovery LLC is seeking a qualified and motivated Case Manager to support individuals receiving substance use disorder treatment services. This position provides coordinated service support to assist clients in achieving stabilization, engaging in treatment, accessing community resources, and developing the skills necessary to sustain long-term recovery and independent functioning.
The Case Manager works collaboratively with multidisciplinary treatment teams, community providers, and recovery support systems to ensure continuity of care and reduction of barriers that may interfere with treatment engagement and recovery progress.

Essential Duties and Responsibilities
- Provide case management services to individuals receiving substance use disorder treatment
- Assist clients in accessing housing, employment, healthcare, transportation, benefits, and community resources
- Coordinate care with clinical staff, peer supporters, medical providers, and community agencies
- Monitor client progress toward treatment and recovery goals
- Participate in interdisciplinary treatment team meetings
- Assist with discharge planning and transitions between levels of care
- Provide support during client crises within scope of role
- Maintain professional boundaries and utilize trauma-informed engagement practices
- Complete timely and accurate documentation in accordance with agency policy and regulatory standards, including Medicaid and Ohio behavioral health requirements
- Maintain knowledge of community resources and recovery supports

Minimum Qualifications
Required:
- Active Chemical Dependency Counselor Assistant (CDCA) credential in the State of Ohio
- High School Diploma or GED
- Valid driver’s license and reliable transportation
- Ability to pass background screening and drug screening
Preferred Qualifications
- Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or related field
- Experience working in substance use treatment or behavioral health settings
- Experience coordinating services for high-risk or vulnerable populations
- Knowledge of ASAM levels of care and recovery-oriented systems
- Experience with Medicaid documentation or behavioral health service coordination
- Familiarity with community behavioral health and social service systems
Work Environment
This role includes a combination of service coordination and community-based client engagement. The position requires flexibility, sound professional judgment, and the ability to effectively support individuals with complex behavioral health and social needs.

Pay: $17.00 - $20.00 per hour
