Role Description
This is a full-time, on-site Case Manager role based in Lexington, KY. The Case Manager will support children in foster care and independent living by providing case management services that include creating individualized service plans, facilitating communication between families, caregivers, and service providers, and monitoring progress. Additional responsibilities include conducting regular home visits, ensuring compliance with policies and licensing requirements, and advocating for the wellbeing and safety of children. The Case Manager will also provide support in the transition process for children moving into independent living programs.
Qualifications
- Case management, care coordination, and client advocacy skills
- Experience in fostering relationships, conflict resolution, and communication with families, caregivers, and external partners
- Ability to assess individual needs, create service plans, and monitor progression for children in foster care or independent living programs
- Strong organizational, time management, and documentation skills
- Familiarity with foster care, independent living requirements, and relevant child welfare policies
- Bachelor’s degree in Social Work, Psychology, or a related field is required
- Licensure as a Certified Social Worker (CSW) or Licensed Social Worker (LSW), or the ability to obtain licensure
- Demonstrated sensitivity to diverse backgrounds, cultures, and experiences
- CPR and First Aid certification are preferred
- Ability to pass required background checks and possess a valid driver’s license
