Position Summary
This position is responsible for transporting residents to various appointments.
Essential Duties
1. Coordinate appointments with the activities department to avoid scheduling conflicts.
2. Review schedule for the coming day and communicate in writing with the nurses station, including time the resident must be ready for the appointment.
3. Before leaving the facility for an appointment, ensure all necessary paperwork is complete and that you are in possession of such, that the resident is presentable for the appointment, and that the resident is dressed appropriately.
4. Check with nurses station 30 minutes prior to departure to assure resident will be ready. This may be done via telephone.
5. Carry cell phone on all transports. Do not talk on the phone while driving; pull over to a safe area to make or answer calls.
7. Ensure the facility van is kept clean and filled with fuel.
8. Maintain mileage/transport log for van.
9. Enforce the no-smoking rule in the van.
10. Fill in at the reception desk when needed.
11. Assist in the activities department when needed.
12. Performs any other duties as assigned by the Administrator.
Basic Qualifications & Experience
1. High school diploma or equivalent.
2. Active CNA License-Preferred
3. Active CPR Certification
4. Valid Driver's License. Excellent driving record with no moving violations or accidents within the last three years.
5. Good communication skills.
6. Ability to assist residents in wheelchairs

If the position applied to is not located in Colorado, the following information may not apply. Pay Range Minimum: $22.28 per hour; Maximum:$24.35 per hour. The base range represents the low and high end of the pay range for this position. Actual pay will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. The range listed is just one component of our total compensation package for employees.
