Social Worker

Reports to: Administrator
Job Duties:
- Resident admissions: Marketing, orientation to the facility, pre-admission screening and coordination, admitting into Point Click Care, Advance Directives, Resident Rights, Discharge Planning.
- Individualized Service Delivery: Care Planning and MDS Assessments (specifically the BIMS, social assessment, and PASSR), identifying social and emotional needs, documentation, resident plan of care for psycho/social needs, social service progress notes.
- Interventions: counseling, crisis intervention, working with special needs residents, advocacy.
- Liaison to support services: psychologist, psychiatrist, podiatrist, dentist, audiology, and optometrist
- Liaison to financial services: Assist family with social programs applications, such as Medicaid.
Educational Requirements:
- Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
- Preference for at least one year experience as a social worker in a skilled nursing facility.
