What You'll Be DoingDeliver Exceptional Resident Care
- Provide skilled nursing care throughout the evening shift
- Perform resident assessments and identify changes in condition
- Administer medications and treatments according to physician orders
- Support residents living with chronic medical conditions, cognitive impairment, and memory loss
- Maintain accurate clinical documentation and medication records
- Promote resident comfort, dignity, safety, and independence
Support Your Team
- Serve as a clinical resource for caregivers and medication staff
- Communicate effectively with physicians, pharmacies, therapists, families, and leadership
- Provide coaching, education, and support to caregivers when needed
- Help maintain a calm, organized, and welcoming environment throughout the community
Help Improve Resident Outcomes
- Recognize subtle clinical changes and intervene proactively
- Support continuity of care and individualized service plans
- Partner with interdisciplinary team members to provide exceptional resident-centered care

What We're Looking ForRequired Qualifications
- Active Minnesota Licensed Practical Nurse (LPN) license in good standing
- Experience in Assisted Living, Long-Term Care, Skilled Nursing, Rehabilitation, Home Health, Hospice, or related healthcare settings preferred
- Strong clinical assessment and nursing skills
- Ability to work independently while exercising sound clinical judgment
- Excellent communication and relationship-building abilities
- Compassionate, dependable, and resident-focused approach to care
- Passion for serving older adults with dignity and respect
