Nursing degree, active registered nurse (RN) license and minimum 1 years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered.

Qualifications and Experience:

• Current and unrestricted RN license
• Previous experience in Home Health or similar community-based nursing care
• Strong assessment and critical thinking abilities
• Proficiency in developing and implementing individualized care plans
• Exceptional communication and documentation skills
• Reliable transportation and valid driver’s license

Key Responsibilities:

• Conduct in-home assessments to evaluate patient needs, safety, and care preferences
• Develop and maintain comprehensive care plans tailored to each patient’s condition and goals
• Administer medication, wound care, and prescribed treatments as directed by healthcare providers
• Educate patients and families on managing health conditions, medications, and prevention strategies
• Monitor and document progress, updating care plans as needed in response to changes
• Coordinate with interdisciplinary teams to ensure continuity and quality of care
• Maintain accurate records in compliance with healthcare regulations and standards

$1,573 - $1,664/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
