The Day Plan Coordinator is responsible for developing, implementing, and monitoring individualized day service plans for adults with intellectual and developmental disabilities served by the agency. This role ensures that each person’s goals, supports, and daily activities align with state standards, person-centered practices, and agency policies.
The coordinator works closely with families, case managers, direct support staff, and community partners to promote meaningful engagement, skill development, and community inclusion.
Associate's Degree in Human Services or related field OR High School Diploma accompanied by two years experience working with individuals with developmental and related disabilities.
Job Type: Full-time
Pay: $17.00 - $19.00 per hour
