- Provide individualized occupational therapy services in patients’ homes
- Evaluate functional abilities and develop patient-centered treatment plans
- Address ADLs, safety, mobility, and independence within the home environment
- Collaborate with physicians, caregivers, and interdisciplinary clinical teams
- Support patients in achieving meaningful, functional outcomes
- Graduate of an accredited Occupational Therapy program
- Active Colorado Occupational Therapist license in good standing
- Home health experience preferred (but not required)
- Strong communication skills and ability to work independently
