What You'll Do
- Provide personalized OT services in patients' homes
- Create functional, individualized treatment plans
- Perform home safety assessments and recommend adaptive tools
- Collaborate with nurses, PTs, caregivers, and families
- Document progress with support and EMR training provided
- Travel locally within Mesa County (mileage and drive time reimbursed)
- Active Colorado OT license (required)
- Home health experience preferred but not required
- Strong communication and autonomous time management
- Reliable vehicle, valid driver's license, and auto insurance
- Passion for helping people stay safe and independent
- Competitive Pay: $48–$52/hour DOE
