What You'll Do
- Provide high-quality, skilled nursing care to patients in their homes
- Perform assessments, develop and update individualized care plans
- Collaborate with a multidisciplinary team of RNs, LPNs, therapists, and caregivers
- Educate patients and families on treatments, medications, and home care techniques
- Monitor progress and maintain accurate documentation in our EMR system HomeCare HomeBase Platform!
- Participate in on call rotation
- Competitive pay: $35–$42/hour (DOE)
- Consistent weekday schedule: Monday–Friday, 8 AM – 5 PM rotating on-call
- Mileage reimbursement for all patient-related travel

- Active Colorado RN license (required)
- Home health experience preferred, but we're happy to train the right candidate
- Strong assessment, clinical, and critical thinking skills
- Excellent communication and documentation abilities
- Reliable vehicle, valid driver's license, and current insurance
