Position Overview
We are seeking Board-Certified or Board-Eligible Family Medicine or Internal Medicine Physicians for immediate openings in the Fort Collins, Loveland, Greeley and Windsor, CO areas. The ideal candidate will provide compassionate, comprehensive care across post-acute and long-term care settings and collaborate closely with interdisciplinary teams.
Key Responsibilities
- Provide ongoing medical care and oversight to assigned patients in skilled nursing, subacute, assisted, and independent living settings.
- Create and maintain timely, appropriate, and confidential documentation in line with medical and billing standards.
- Participate in care coordination with therapists, specialists, and facility staff.
- Meet quality, safety, and regulatory benchmarks for clinical care and documentation.
- Support practice-wide utilization and care management goals.
- Engage in peer reviews, quality improvement, and professional development activities.
- Build positive relationships with patients, families, facility staff, and the Summit team.
- Travel between multiple facilities as needed; reliable transportation required.
- Share minimal rotating weekend and holiday call duties (no night call).
Essential Qualifications
Required:
- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in Family Medicine or Internal Medicine.
- Current and unrestricted medical license in Colorado or eligibility to obtain one.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Reliable transportation.
- Minimal weekend call and holiday coverage
Preferred:
- Familiarity with Point Click Care, eClinicalWorks and Google Suite.
- Experience in post-acute care, skilled nursing, or hospitalist settings.
