RN Hospice Case Manager

We are seeking a compassionate and dedicated hospice nurse to join our team and make a meaningful difference in the lives of patients and families. In this role, you will provide high-quality nursing care to individuals with terminal illnesses, ensuring comfort, dignity, and peace throughout their end-of-life journey.

As part of an interdisciplinary team, you’ll assess patient needs, develop and implement individualized care plans, administer medications, and manage symptoms with empathy and clinical skill. You’ll also support families with education, guidance, and emotional care, helping them navigate this deeply personal time with confidence and comfort.

This position includes on-call responsibilities and requires a nurse who is flexible, responsive, and dependable—able to bring calm reassurance and expert care at any hour. Your ability to act quickly and compassionately makes a lasting impact when patients and families need it most.

A strong background in patient care, hospice case management is highly preferred. You must be confident working independently across various care settings, including homes and outpatient environments.

Thorough and timely documentation is key—not only for compliance but for ensuring consistent, coordinated, high-quality care. Accurate charting reflects your clinical excellence and supports the entire care team in delivering the best possible outcomes.

This position serves patients in Kyle, Buda, San Marcos, New Braunfels area.
