- Valid New York State HHA Certificate
- Government-issued photo ID & Social Security Card (or Work Authorization, if applicable)
- Two reference letters
- Recent physical exam
- TB screening (Quantiferon preferred)
- Proof of flu vaccination
- Rubella & Rubeola titers
- Chain of Custody drug screening (within the last 6 months)
