Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care. Summit is also proud to offer AI-Assisted clinical documentation to streamline charting and free ECUs.
Physical Therapist (PT):
- Full-Time Hours: 40 Hours/Week (Flexible Scheduling); Minimum 25 Visits/Week (7 Points/Day)
- Pay Structure: $95,000 - $115,000/year (Based on Pay per Visit Rates and Experience)
- Sign on Bonus: $5,000 sign on bonus
- Additional Earnings: $2,000 Quarterly Bonus for Maintaining 90% Average Productivity
- Coverage Location: Jasper, Indiana & Surrounding Areas (Including Washington, IN)
- Travel: Required Milage Reimbursement ($0.48/Mile)
- Work Setting: In-Home Patient Visits, Training On-Site at Our Local Office and Remotely)
Responsibilities:
- As a PT, you’ll play a vital role in helping patients regain mobility and independence
- Delivering physical therapy services per physician’s care plan and state regulations
- Assessing patient mobility, strength, and rehabilitation potential
- Designing and implementing personalized therapy programs
- Educating patients and caregivers on therapeutic techniques and equipment
- Collaborating with interdisciplinary teams to ensure holistic care
- Supervising PTAs and home health aides as needed
- Documenting progress and participating in discharge planning
What We're Looking For
Required:
- Active Indiana PT license
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
- CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Recent TB test
Preferred:
- 2 years of PT experience
- Home health or community-based care experience
- Strong communication and organizational skills
- Ability to build rapport and lead with empathy
