Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.
Part-Time Physical Therapist (PT):
- Part-Time Hours: Monday - Friday; ~20 Hours/Week (Flexible Scheduling)
- Coverage Location: Paoli, Indiana & Surrounding Areas (Including French Lick, IN)
- Travel: Required Milage Reimbursement
- Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
- Pay Structure: Pay per Visit (Rates Vary Depending on Service, Experience & Coverage Area)
Responsibilities:
- As a PT, you’ll play a vital role in helping patients regain mobility and independence
- Delivering physical therapy services per physician’s care plan and state regulations
- Assessing patient mobility, strength, and rehabilitation potential
- Designing and implementing personalized therapy programs
- Educating patients and caregivers on therapeutic techniques and equipment
- Collaborating with interdisciplinary teams to ensure holistic care
- Supervising PTAs and home health aides as needed
- Documenting progress and participating in discharge planning
What We're Looking For
Required:
- Active Indiana PT license
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
- CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Recent TB test
Preferred:
- 2 years of PT experience
- Home health or community-based care experience
- Strong communication and organizational skills
- Ability to build rapport and lead with empathy
