This position is responsible for providing families of eligible children developmentally appropriate strategies using the Evidence Based Early Intervention model. Must strive to meet the Agency mission and exemplify the Agency’s core values.
Education / Experience:
Bachelor’s or graduate-level degree in education, health, social or behavioral science, social services, human services, or other related field or
Bachelor’s or graduate-level degree in unrelated field and two (2) years full-time*, supervised, paid experience working with children birth through age 5 with disabilities, developmental delays, or diagnosed physical or mental conditions with high probability of resulting in a developmental delay and their families or
Valid license in counseling; early childhood education for ages 3 through 8, grades kindergarten through third, or special education; hearing impairment invention; medicine; nursing; occupational therapy; physical therapy; psychology; psychiatry; social work; speech-language pathology; or visual impairment intervention.
Skills: Proficiency in Microsoft Office and database programs. Use mathematics to solve problems. Obtain and use equipment, facilities and materials required to perform work. Operate job related equipment including the use of basic computer programs and systems. Do data entry, generate reports, copy, manage and track documents and information.

Knowledge: Knowledge of behavior support, person centered planning, DD programs, services and community resources. General principles and objectives of information technology systems and programs. Proper spelling, grammar, punctuation, and sentence structure to ensure that written materials prepared and reviewed are complete, succinct, and free of errors. Principles, practices and methods of safety and health as it relates to persons served, employees and the public. Gather, analyze and interpret data.

Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to:
1. Works together with the family/caregiver to identify the family’s/caregiver’s priorities and concerns for their child.
2. Develops individualized, evidence-based strategies that can easily be incorporated into the child’s/family’s/caregiver’s existing activities and daily routines which promote active participation of caregivers, children, materials, and the specific environment that leads to achieving IFSP outcomes.
3. Functions as a Primary Service Provider (PSP) to deliver special instruction in the child’s home or community setting.
4. Participates as a member of a trans-disciplinary team; attends weekly team meetings, IFSP meetings, and Transition Planning Conferences.
5. Maintains open communication with the families/caregivers to ensure quality services are being delivered.
6. Conducts developmental assessments in the five developmental domains to plan service needs.
7. Participates in developing and writing IFSP outcomes and strategies with the family/caregiver and Early Intervention Service Coordinator.
8. Supports community agencies to include children with disabilities.
9. Participates in meetings, in-services and / or training as representative of Summit DD internally as well as within Summit County and beyond.
10. Perform other related duties as assigned.
