What are the responsibilities and job description for the Patient Service Representative -School Based Health Center (Park County) position at Summit Community Care Clinic, Inc.?

The Park SBHC Patient Service Representative (PSR) is the first point of contact between the student and their families and our School Based Health Center with responsibilities for supporting the student and their families either in person or via the phone, a welcoming voice and offering a professional invitation into our clinic. This will include scheduling, patient outreach, and ensuring care is coordinated and integrated between all three service lines (medical, dental, and behavioral health). The Park SBHC PSR is located at the School Based Health Center in the school’s administration building in Fairplay, CO. The Park County School District holds classes Monday – Thursday each week. During non-school hours, the PSR may be asked to work at the administration offices at the main Care Clinic in Frisco.


Essential Functions:

  • Operations: Transitioning the Patient from the point of contact to their appointment, through to patient checkout. Maximizing patient scheduling opportunities with attention to operational flow and the needs of the patient.
  • Screen all new and established patients for eligibility for all School Based Health Services.
  • Patient Appointment Preparation: Prepare patient records for appointment readiness. Gather required paperwork and update demographics, insurance, enrollment for accurate records completion in the Electronic Records system.
  • Responsible for filling schedules, scrubbing, and reminder calls for the Park County School District.
  • Collect payment and complete end of day reconciliation.
  • Participate in school and community events including, but not limited to, back to school nights, school registrations, and SBHC immunization clinics.
  • Lead and participate in team huddles.
  • Provide administrative support for integrated care.
  • Other duties as assigned by SBHC Director.

Additional Requirements:

  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required trainings as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.
  • Must participate in quality assurance/ improvement activities when applicable.

Education/Experience:


Must Haves to be successful:

  • Associates or bachelor’s degree or equivalent work experience.
  • Proficient customer service skills preferably healthcare but not required.
  • Strong written and verbal communication skills.
  • Basic knowledge of digital systems (including Microsoft Office Suite) is required.
  • Critical level of attention to detail, organizational skills, and time management.
  • Ability to read, write, and speak the English language.

Nice to Haves:

  • Ability to speak the Spanish language preferred (bilingual).

Work Environment:

The work environment is in a healthcare setting which can be fast paced and involves working directly with patients and healthcare professionals. The environment is primarily clinical, but the individual may work in a clerical setting. Many of our patients with chronic diseases have a minimal level of health literacy and a significant number of barriers to accessing health care.
