Physical Therapist (PT)
Summa Health at Home and Hospice is seeking a compassionate Physical Therapist (PT) to join our home health team.
$10,000 SIGN-ON BONUS!
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Territory: Macedonia, Twinsburg, Aurora, Brady Lake, Garrettsville, Hiram, Hudson, Kent, Streetsboro, Cuyahoga Falls, and surrounding areas

What You’ll Do
- Provide skilled physical therapy services in patients’ homes as ordered by the physician.
- Evaluate patients, develop individualized plans of care, and adjust treatment based on progress and changing needs.
- Treat patients to reduce pain, restore function, and maximize independence and mobility.
- Educate patients and families on treatment plans, exercises, equipment, safety, and disease/injury management.
- Complete OASIS assessments, evaluations, admissions, and required clinical documentation.
- Communicate patient progress and changes with physicians, families, and the interdisciplinary care team.
- Help reduce hospitalizations through education, proactive care, and appropriate visit planning.
- Supervise PTAs and other therapy support staff as appropriate.
- Participate in case conferences, on-call coverage, required trainings, and team meetings.
Qualifications
- Current Physical Therapist license in the state of employment.
- Minimum 1 year of acute-care Physical Therapy experience required.
- Home health experience preferred.
- Strong communication, organization, clinical judgment, and problem-solving skills.
- Ability to work independently while collaborating effectively with patients, families, physicians, and coworkers.
- Valid driver’s license and auto insurance with the applicant listed as a driver.
Physical Requirements
This is a field-based position requiring regular travel to patient homes. The role involves walking, standing, sitting, bending, reaching, driving, and lifting up to 50 pounds with or without assistance. Reasonable accommodations may be available for qualified individuals.
