Job Overview
The Director of Case Management is responsible for leading and overseeing the hospital’s case management department. This role ensures patients receive safe, timely, and well-coordinated care from admission through discharge.
The Director works closely with hospital leadership, clinical teams, and outside agencies to ensure smooth discharge planning, compliance with regulations, and effective use of clinical resources.
This is a leadership role that requires strong communication skills, decision-making ability, and experience managing hospital case management operations.
Key Responsibilities
- Lead and manage all daily operations of the Case Management Department
- Ensure compliance with hospital policies and regulatory requirements
- Oversee patient discharge planning and continuity of care
- Collaborate with interdisciplinary hospital teams and external agencies
- Improve patient outcomes and efficient use of hospital resources
- Provide leadership, coaching, and support to case management staff
- Manage change, resolve conflicts, and guide department improvements
Required Qualifications
- Bachelor’s degree (required)
- Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) (required)
- 3 years of recent acute care hospital case management experience
- 2 years of leadership experience in case management
- Hospital discharge planning experience (required)
