Location: Sullivan Park Care Center
Position Summary
Sullivan Park Care Center is seeking dedicated and detail-oriented Licensed Practical Nurses (LPNs) to join our Medicare team.
Key Responsibilities
- Complete and follow up on MD orders in collaboration with the interdisciplinary team.
- Document daily skilled nursing notes in accordance with Medicare guidelines.
- Perform and record weights and nutrition at risk.
- Conduct comprehensive resident evaluations, ensuring timely assessments and updates.
- Lead and participate in infection prevention protocols, including monitoring, reporting, and staff education.
- Maintain compliance with all federal, state, and facility policies, procedures, and documentation standards.
Qualifications
- Current LPN license in the state of Washington.
- Prior experience in skilled nursing, long-term care, or Medicare documentation strongly preferred.
- Strong clinical assessment, organizational, and time-management skills.
- Knowledge of MDS processes and Medicare regulations is a plus.
- Ability to work independently and as part of a team.
- Excellent communication and documentation skills.

Pay: $38.50-$43.50 per hour depending on experience and education.
Salary : $480,000 - $510,000
