Key Responsibilities
Phone Triage & Clinical Support
- Serve as the primary point of contact for hospice patients and families during weekend hours.
- Conduct thorough symptom assessments over the phone and provide clinical guidance, education, and emotional support.
- Determine when an in-person nursing visit or escalation is necessary.
- Coordinate care with the interdisciplinary hospice team, including physicians, aides, and case managers.
- Document all triage calls, interventions, and follow-up actions in the EMR promptly and accurately.
- Respond to urgent patient needs that require in-person evaluation or intervention.
- Administer medications, manage symptoms, and provide comfort measures consistent with hospice protocols.
- Communicate changes in patient condition to the appropriate on-call or case management staff.
- Graduate of an accredited School of Nursing
- Active RN license in the state of Minnesota
- Minimum 1 year of hospice or home health experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, calm, and patient-centered approach
- Reliable phone and internet connection for remote triage duties
- Valid driver's license and insured vehicle for occasional visits
- CPR certification required
- Daily per diem rate for weekend phone coverage
- Hourly rate for any in-person visits conducted
- Mileage reimbursement for travel when applicable
