Licensed Practical Nurse (LPN)
Full-Time, Exempt (40 hours/week)

As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality, resident-centered care within our Assisted Living and Memory Care communities. You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure each resident receives safe, compassionate, and personalized care.

Provide direct nursing care, including medication administration, treatments, and assessments

Monitor resident health and report changes promptly to the care team

Assist in developing and implementing individualized care plans

Support residents with memory loss through patience, empathy, and engagement

Build strong relationships with residents, families, and team members

Maintain accurate and timely clinical documentation

Mentor and support caregivers and new staff

Ensure compliance with state regulations and company policies

Graduate of an accredited practical nursing program
Licensure

Active Minnesota LPN license in good standing
