Job description:
The Aging in Place NNORC (Neighborhood Naturally Occurring Retirement Community) Social Worker for Hands on Huntington will provide direct social work services and coordinate an interdisciplinary team of professionals and volunteers to implement services to help seniors to age-in-place in the community. This position includes both direct service provision and administrative responsibilities in managing both the program and staff.
Essential Responsibilities:
- Conduct in-home intake assessments of NNORC residents interested in NNORC services in collaboration with the NNORC Nurse;
- Provide case management for a case load of NNORC residents, including assessment and reassessment, care planning, case coordination, collateral contacts, monitoring, linkages, and benefits counseling/application;
Position Type/Expected Hours of Work:
Full time, 40 hours/week.
Required Education and Experience *
- Demonstrated expertise in counseling, crisis intervention, and case management
- Strong skills in community engagement, social work planning, and implementation
- Ability to assess and address individual and family needs in diverse settings
- Proficiency in maintaining accurate records, documentation, and reporting
- Excellent communication, active listening, and collaboration skills
- Proficient understanding of mental health and emotional well-being support
- Master's degree in Social Work (MSW) and valid LMSW licensure in New York State
- Five years relevant experience (may include experience gained before completing masters).
Knowledge and Skills:
- Psychosocial assessment
- Group work and program planning
- SIFI completion (or eligibility), for supervision of social work interns
- Familiarity with psychosocial issues related to aging, including: bereavement, changes in personal health status, caregiving, adjustment to retirement, changes in family dynamics
- Familiarity with basic entitlements and community resources
- Familiarity with Microsoft Word and Excel
