Success Rehabilitation, Inc. is dedicated to providing compassionate, specialized care for individuals rebuilding their lives after brain injuries. We are seeking a detail-oriented, proactive Client Health & Wellness Assistant to join our team in Quakertown.
In this role, you will split your time between providing high-quality direct care to program participants and delivering vital administrative support to our Health and Wellness Department. What You'll Do
- Medication Administration: Safely administer prescribed medications to clients following Pennsylvania DHS guidelines. Accurately document dosages, timing, and client identity in our Electronic Medication Administration Record (EMAR) system.
- Clinical Administrative Support: Maintain client medical charts, log vaccine data, scan medical summaries, and audit routine wellness or preventative care visit spreadsheets.
- Pharmacy Coordination: Manage weekly medication refills, reconcile shipments, handle expired/discontinued medications, and communicate updates directly with pharmacies and physician offices.
- Client Monitoring & Care: Take vital signs, monitor clients for unexpected medication reactions or illness, and communicate health updates promptly to the Health & Wellness Coordinator.
- Inventory Management: Conduct monthly inventories of the medical supply closet and medication office. Order and track Personal Protective Equipment (PPE) and daily administration supplies (med cups, spoons, etc.).
- Staff Tracking & Coordination: Monitor staff due dates for medication administration and insulin training, send reminders, and assist in scheduling classes and reviews.
- Transportation & Outings: Safely transport program participants to scheduled medical appointments using company vehicles in the local Quakertown and surrounding communities.
What We’re Looking For
- Certifications: Must pass the PA DHS Medication Administration test and maintain active certification (or be willing to obtain upon hire). Active CPR/First Aid certification is required.
- Licensure: A valid driver’s license with a clean driving record is required.
- Background Checks: Must pass a comprehensive criminal history check and Pennsylvania Child Abuse History Clearance (Act 33).
- Technical Skills: Basic computer skills using Microsoft Word, email systems, and the ability to learn Electronic Health Records (EHR) / EMAR systems.
- Core Competencies: Impeccable attention to detail, strong verbal and written communication skills, and the ability to collaborate effectively with clinical leadership, residential teams, and direct-care staff.
Physical Demands & Working Conditions
- Lifting & Transfers: Ability to independently perform client transfers (including maximum assistance transfers) and lift, push, pull, or move a minimum of 50 lbs.
- Mobility: Regular walking, standing, and sitting for up to 8 hours per shift. Ability to successfully complete physical requirements for Red Cross First Aid/CPR courses.
- Work Environment: Time is split between our temperature-controlled outpatient facility and our integrated residential apartments/townhouses.
- Travel: Occasionally transporting clients in company vehicles across the community for medical appointments in all types of weather.
