What are the responsibilities and job description for the Hospice and Palliative Care Nurse Practitioner position at Suburban Hospice, Inc.?
The Hospice and Palliative Care Nurse Practitioner (NP) provides compassionate, comprehensive care to patients with life-limiting illnesses. This role includes clinical assessment, symptom management, advanced care planning, and coordination of services to enhance quality of life. The NP works collaboratively with the interdisciplinary team, patients, and families to deliver patient-centered care in various settings, including homes, hospitals, skilled nursing facilities, and hospice inpatient units.
The nurse practitioner works in collaboration with the Administrator and Hospice interdisciplinary team to provide Hospice and Palliative care services to patients and families including:
Clinical Assessment & Care:
- Conduct physical exams, order and interpret diagnostic tests, and develop individualized care plans.
- Manage symptoms such as pain, dyspnea, nausea, and anxiety using pharmacologic and non-pharmacologic strategies.
- Provide palliative and hospice consultations in various care settings.
Care Coordination:
- Collaborate with physicians, nurses, social workers, chaplains, and other team members to coordinate holistic care.
- Facilitate transitions of care, including hospice enrollment and discharge planning.
- May be required to perform on-call on week nights and weekends as needed.
Communication & Support:
- Lead and participate in family meetings to establish goals of care and discuss prognosis and treatment options.
- Provide emotional support and counseling to patients and families.
- Educate families about disease progression, hospice philosophy, and expectations.
Documentation & Compliance:
- Maintain accurate and timely electronic medical records (EMR) in compliance with state, federal, and organizational standards.
- Ensure compliance with Medicare/Medicaid regulations and hospice guidelines.
Professional Development:
- Stay current with evidence-based practices in hospice and palliative medicine.
- Participate in quality improvement projects, team meetings, and continuing education.

Must be a Nurse Practitioner (NP) by the Indiana Professional Licensing Board
- Previous experience in hospice, palliative care, or a related field required
- Minimum 1–2 years of experience in hospice, palliative care, oncology, geriatrics, or related field preferred
