What are the responsibilities and job description for the Telephone Triage Nurse position at Suburban Health at Home?
The Triage Nurse responds to all patient requests telephonically during on-call hours (4:30 pm-8:00 am) by providing professional nursing assessment to identify the urgency of patient needs.
- Interviews the patient and/or family to identify triage priority based on available information
- Calls for crisis intervention if appropriate
- Communicates and acts as a liaison with patients, family and other health professionals to ensure the needs of the patient are adequately addressed
- Provides education to the patient and/or family in accordance with the Plan of Care
- Responds to patient calls within 15 minutes
- Accurately documents all on-call activities via the on-call communication log
- Contacts physicians if PRN orders are needed in order for the on-call nurse to perform a call-back visit
- Contacts the regional on-call nurse and provides report when call-back visits are warranted
- Consults the Administrator on-call when necessary
- Adheres to agency documentation requirements to provide timely, detailed, accurate documentation of care or services provided
- Graduate of an accredited school of professional nursing.
- Minimum of 3 years nursing experience in an acute care setting, preferably home health.
- Minimum of 1 year home health experience
- Must possess current license from the Indiana State Board of Nursing.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Must possess attention to detail skills.
- Ability to prioritize work using fundamental time management skills.
- Must possess excellent interpersonal skills to foster a team environment.
