We're looking for a Social Worker / Case Manager who is passionate about improving the lives of older adults. In this role, you'll work directly with residents and partner closely with our intake coordinator and clinical team to ensure seamless, coordinated care.

We're looking for a Social Worker / Case Manager who is passionate about improving the lives of older adults. In this role, you'll work directly with residents and partner closely with our intake coordinator and clinical team to ensure seamless, coordinated care.
What You'll Do
- Support residents by coordinating all aspects of their care.
- Assist with required paperwork, insurance coordination, DME requests, scheduling, and other non-clinical needs.
- Communicate regularly with residents, families, and facility staff.
- Use our EMR system to keep information organized and up-to-date.
- Travel between assigned facilities as needed.
- You genuinely enjoy working with the elderly population.
- You're well-organized, comfortable on the phone, and confident using EMR systems.
- You have previous experience in case management or a healthcare setting (highly preferred).
- A BSW is a plus—but not required.
