What are the responsibilities and job description for the Registered Nurse position at SUBURBAN ADULT SERVICES INC.?
SUMMARY OF DUTIES:
- Responsible for implementing State and agency regulations, policies and procedures.
- Oversees and advises individuals receiving support and staff in health-related issues.
- Responsible for ensuring that confidentiality is strictly maintained concerning agency and individual related information and that HIPAA/HITECH policies and practices are adhered too.
ESSENTIAL DUTIES:
1. Provides nursing services to persons admitted to assigned programs.
2. Develops and delivers health services based on the interdisciplinary teams' recommended intensity, duration and frequency, in accordance with general acceptable community standards of professional practice.
3. Works with the interdisciplinary team and, as needed, other agency staff to ensure coordination of service development.
4. Documents in each participant’s confidential or clinical record all diagnostic assessment reports, individual plans of treatment and treatment notes regarding services being recommended and/or delivered.
5. Consults with each person receiving services and/or collaterals regarding treatment outcomes and course of treatment, and provides information as required to Treatment, Residential and Program Directors, Medicaid Service Coordinators and other agency staff as indicated for service coordination and follow up.
6. Trains, coaches and certifies staff in medication administration, tube feeding, and insulin injections as per OPWDD regulations and directives, and agency policies.
a. Monitors and observes staff’s performance of above activities for certification purposes and as necessary to ensure competence.
b. Reviews medication administration records to ensure accuracy and completeness as per agency policy.
c. Identifies and addresses errors related to the above as per agency policies, including decertification of staff as appropriate.
d. Supports management/administrative staff in supervisory follow-up of errors/ decertification as appropriate.
e. Trains, coaches, and provides programmatic supervisor of staff in the performance of any other nursing related tasks.
f. Develops, reviews and trains staff on individual Plans of Nursing Services.
7. Provides direct nursing services as needed or scheduled in Respite Services.
8. Ensures medically related documentation and information relative to individuals receiving support is accurate, complete and properly filed as per agency policy and practice.
9. Ensures all communications with individuals receiving supports and their families/advocates/ guardians, direct support staff, management, administration, other nurses, medical providers, etc. is respectful, clear, professional and understandable to the other person as much as possible.
10. Ensures protected health information is handled in compliance with HIPAA.
11. Ensures appropriate follow-up to medical appointments.
12. Monitors infection control of assigned sites.
13. Participates in quality assurance activities as required.
14. Accompanies individuals receiving supports on medical appointments, as deemed necessary.
15. Responsible for 24/7 on-call when assigned.
16. Other duties as assigned.
OTHER DUTIES:
17. Ensures the Exposure Control Plan is followed in instances of staff/ individual receiving supports exposure to blood or other potentially infectious materials.
18. Assists in the establishment of new programs as directed.
19. Provides TB testing and maintains confidential medical information on employees and volunteers.
20. Reviews and advises appropriate nutrition and exercise for individuals receiving support.
21. Assists administrative team in updating policy and medically related information and training.
QUALIFICATIONS:
- Be at least 18 years old
- Hold a valid driver’s license – to travel to assigned locations throughout the work day, and

EDUCATION & EXPERIENCE:
Bachelors or Associates Degree in Nursing
CERTIFICATIONS/LICENSES & REGISTRATIONS:
- Must be licensed and currently registered with New York State Department of Education as a registered nurse or nurse practitioner.
LANGUAGE SKILLS:
- Be able to communicate clearly both in person and on the phone.
- Be able to listen to what other people are saying and ask appropriate questions.
- Be able to read and interpret documents such as habilitation notes, medical reports, and procedure manuals.
- Be able to type and use a computer to complete a variety of documents and reports.
- Be able to write well, with good spelling, grammar, and sentence/paragraph structure.
MATHEMATICAL SKILLS:
- Be able to perform basic math functions (e.g., addition, subtraction, multiplication, and division, and percentages).
REASONING ABILITY:
- Have very good organization and planning skills.
- Be able to solve most daily problems and to obtain the information necessary to solve more complex problems.
- Must be able to objectively evaluate information through critical thinking.
- Must be able to make medical decisions.
- Must be able to organize own time and accomplish objectives with little supervision.
PHYSICAL DEMANDS:
- Must be able to make on-site visits and attend training/meetings.
TRAINING REQUIREMENTS:
- Complete all mandatory and required training within established guidelines and annual updates as required.
- Completion of training offered by OPWDD and others to keep abreast of current laws, regulations, and practices in the field.
Experience
Required- Must be licensed and currently registered with New York State Department of Education as a registered nurse or nurse practitioner.
