We are seeking a dedicated and experienced Social Services Director to join our team at Stroudsburg Post Acute Nursing and Rehabilitation. As a Full-time social worker, you will play a vital role in delivering exceptional care to our residents.
Responsibilities:
- Develop preliminary and comprehensive assessments for each resident, identifying their social problems and needs.
- Collaborate with residents and their families to develop and review individualized plans of care, ensuring their voices are heard and valued.
- Coordinate discharge planning to ensure a seamless transition for our residents back into their communities.
- Create written plans of care that outline goals and objectives for addressing each resident's social problems and needs.
- Ensure timely and efficient completion of social services components in Minimum Data Set (MDS) assessments.
- Review nurse's notes to monitor the effectiveness of care plans and make necessary adjustments.
- Provide exceptional customer service to our residents, families, and staff.
Additional Requirements:
- Bachelor's degree in Social Work, Sociology, or a related field (or Master's degree preferred).
- Minimum 2 years of experience in social work, preferably in a healthcare setting.
- Strong communication, assessment, and planning skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
