Stroudsburg Post Acute Nursing and Rehabilitation is looking for a reliable and devoted Occupational Therapist to implement treatment plans and provide support to skilled and short-term residents. Such treatment must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Rehabilitation to ensure that the highest degree of quality care is always maintained. SummaryEvaluates, plans, directs, and administers occupational therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence in a skilled nursing setting. Provides assessment and treatment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning patients to their prior level of function and or maintaining maximum potential. Improves and/or facilitates quality of life through prescribed exercise, hands-on care, and patient education.Job DutiesEstablishes and implements appropriate occupational therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.Provides occupational therapy treatment consistent with the patients' needs assessment.Performs reassessment when indicated and modifies occupational therapy plans accordingly.Communicates patient progress and physical limitations to supervisor, physician(s), and other team membersProvides supervision in accordance with regulatory requirements, as appropriate.Establishes an occupational therapy treatment plan with measurable goals and time frames.Completes and submits all documentation based on policies/standards and regulatory requirements.Preferred QualificationsMaster’s Degree Physical Therapy or Doctorate Degree Physical Therapy
