Description:
This position oversees Vocational programs regarding individual care plan, health and safety, and financial matters. Assures individuals receive all services and support identified in the service plan including development, implementation of individual service and support plans; behavior support plans; nursing protocols; therapy programs and protocols; other identified services and supports. Facilitates individual moves including new admissions, inter-agency transfers, and transfers to other providers. Ensures program environments meet all regulatory and Strive standards.
This position supervises staff to ensure they receive orientation and on-the-job training. Facilitates a teaching/learning environment and fosters a supportive and person-centered atmosphere. Routinely observes staff performance and provides feedback, coaching and support.
Requirements:
Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
Oversees Vocational program(s) including clients, individuals and employees assigned to those departments.
Attends meetings, in-services, trainings, etc., on a consistent basis and as needed. Participates in the development of in-service training modules and facilitates in-service training as needed. Participates and requests meetings such as SP/IDT team meetings as needed.
Follow up on emails, phone calls, concerns, etc., in a timely manner, preferably within two business days.
Responds to GER, MANE, and other investigations in a timely manner.
Facilitates a “therapeutic/teaching/learning” environment and fosters a supportive and person-centered atmosphere for individuals attending program/work settings.
Assures individuals have opportunities to exercise choice and pursue personal goals.
Follows all individual service plans to ensure that individuals receive services and supports identified in their Service Plans including development and implementation of Individual Service and Support Plans; Behavior Support Plans; Nursing care plans and protocols; Therapy programs and protocols; and other identified services and supports.
Ensures that individual supervision needs are posted and adhered to.
Participates in the development and implementation of new program initiatives.
Participates in developing and updating of life skills assessments and skill building programs in a timely manner. Ensures staff are engaged with individuals, and individuals are involved in habilitative activities.
Facilitates interviewing for open positions within assigned department.
Provides facility and program orientation and on-the-job training for all staff assigned to department in an accurate and timely manner.
Ensures all program department training is up to date and accurate for all employees in an accurate and timely manner.
Conducts employee evaluations as appropriate to meet HR requirements. Routinely observes staff performance on all shifts and provides feedback, coaching and support.
Promotes teamwork amongst staff and follows up on staff recommendations timely.
Conducts regular meetings to address staff, program, and administrative issues. Consults with Supervisor regarding issues requiring elevated attention.
Reviews GER’s and T-Logs daily. Ensures documentation is objective and professional. Completes follow up for all GER’s as appropriate.
Approves timecards, mileage reimbursement and allocates overtime incurred for assigned employees in an accurate and timely manner. Submits change forms timely.
Completes required organization forms, including but not limited to HR Forms, Payroll Forms, Finance and Accounting Forms, etc., and submits to appropriate department in an accurate and timely manner.
Responds to personnel concerns in a timely manner. Follows staff disciplinary policies and processes and conducts corrective action in cooperation with Human Resources. Documents personnel actions and submits documentation to HR for personnel files as required.
Ensures facilities are maintained in a safe, hygienic, and satisfactory manner, including adherence to all regulatory standards. Addresses and documents efficiencies noted during safety inspections and follows up in a timely manner.
Ensures needed repairs and maintenance are completed satisfactorily and in a timely manner; and ensures furnishings and equipment are maintained in good condition and are in adequate supply on a consistent basis.
Ensures that emergency drills are implemented and documented as required. Ensures facility and community emergency plans are up to date and reviewed by staff and individuals on a quarterly basis.
Maintains vehicle and facility first aid kits and confirms kits are present in department and fully stocked on an on-going basis.
Ensures each vehicle is equipped with community emergency plans.
Serves as medication trainer/qualified manager and provides oversight to staff in medication administration. Investigates and addresses medication errors timely and in accordance with established procedures.
Confirms medication/therapeutic orders are current including, but not limited to, availability of required medications.
Ensures correct documentation of all medical/therapeutic supports. Responsible to follow all health protocols and confirms department staff follow all health protocols.
Assures that staff are knowledgeable about and adhere to individual diets.
Participates in department and program budget development to include monitoring program budgets and maintaining budget according to established limits. Investigates over and under expenditures and implements corrective procedures.
Reconciles P-Card accounts monthly. Assures that staff follow designated P-Card procedures.
Review and approve attendance and transportation billing weekly.
Review and submit community billing on a monthly basis. Addresses noted concerns in a timely manner.
Develops and maintains knowledge of Strive policies, regulatory requirements and licensing standards and assures program compliance.
Participates in surveys by regulatory agencies as needed. Ensures implementation of plans of correction in an accurate and timely manner.
Completes and submits monitoring tools per prescribed schedule, addressing any noted concerns and documenting actions taken. Assures that concerns noted through case management or organizational monitoring are addressed in an accurate and timely manner.
Assists in implementation of strategic plans and provides data for strategic plan metrics as needed. Participates in accreditation activities as needed and assures that staff are provided information related to the strategic plan and accreditation.
Assures that individuals have available to them any prescribed and/or needed adaptive equipment or assistive technology. Assures that individual adaptive equipment is kept clean and maintained in good repair. Notifies therapist or otherwise facilitates repairs to adaptive equipment when problems are noted. Assures that Vocational environments are barrier-free to the maximum degree possible.
Responds to all emergency situations as appropriate and in a timely manner.
Acts as backup for DSP positions as needed to cover open shifts.
Other job-related duties as assigned.
