Responsibilities
- Provide one-on-one, in-home physical therapy services to clients- mostly in Southern Rhode Island, but throughout the state
- Conduct thorough evaluations and re-evaluations to develop and update individualized care plans (DPT)
- Implement treatment plans as directed by the supervising DPT (PTA)
- Deliver evidence-based, goal-driven therapy to improve patient mobility, function, and quality of life
- Document all visits and progress notes accurately and in a timely manner using our EMR system
- Educate patients and caregivers on exercises, home safety, and self-care techniques
- Communicate and coordinate care with other healthcare professionals as needed
- Manage travel between clients’ homes
- Maintain professional, compassionate, and respectful demeanor in all client interactions
Qualifications
- A licensed DPT or PTA in Rhode Island
- Professional, compassionate, and independent
- Comfortable providing outpatient care within a home environment
- Passionate about patient-centered treatment
- Willingness to learn and grow professional skills
