Position Summary

The Case Manager provides coordination of care, advocacy, and support services for clients receiving treatment in a residential substance use treatment facility. The Case Manager assists clients with treatment planning, discharge planning, community resource linkage, and continuity of care to support long-term recovery and stability. This role works collaboratively with clinical staff, medical providers, families, referral sources, and community agencies to ensure clients receive comprehensive and individualized support throughout treatment.


Shift: 9:00am-6:00pm Monday-Friday (business hours)


Job Responsibilities

  • Conduct clients needs assessments and assist in identifying barriers to recovery and stability

  • Coordinate and assist with discharge planning and aftercare arrangements

  • Connect clients with community resources including housing, transportation, employment, education, medical care, psychiatric services, legal assistance, and recovery support services

  • Assist clients with scheduling appointments and completing referrals to outside providers and agencies

  • Collaborate with therapists, medical staff, Behavioral Health Technicians, and leadership regarding client progress and care coordination

  • Maintain regular communication with referral sources, probation officers, family members, and outside agencies as authorized

  • Monitor and document client progress toward treatment and discharge goals

  • Maintain accurate, timely, and compliant documentation within the electronic health record

  • Assist clients with obtaining identification documents, insurance applications, benefits, and other supportive services as needed

  • Participate in multidisciplinary treatment team meetings and case consultations

  • Educate clients on available resources, recovery supports, and community services

  • Advocate for client needs while promoting accountability and independence

  • Maintain confidentiality and comply with HIPAA, facility policies, and ethical standards

  • Assist with transportation coordination and scheduling as needed

  • Participate in trainings, staff meetings, and professional development activities

Qualifications

  • High school diploma or GED required

  • Associate or Bachelor’s degree in Social Worker, Human Services, Psychology, Counseling, or related field preferred

  • Experience working in behavioral health, substance use treatment, social services, or case management preferred

  • Strong organizational, communication, and problem-solving skills

  • Ability to maintain professional boundaries and confidentiality

  • Basic computer and documentation skills

  • Ability to pass background screening and drug testing

  • Valid driver’s license with acceptable driving record

  • Knowledge of substance use disorders, recovery principles, and community resources

  • Experience with discharge planning and care coordination

  • CPR/First Aid certification

  • Case management certification or relevant credential

  • Experience with electronic health record (EHR) systems
