Responsibilities
Dispense medications accurately and efficiently within an LTC pharmacy setting, ensuring adherence to all legal and safety standards.
Conduct thorough patient assessments to develop personalized medication plans that optimize therapeutic outcomes.
Provide clinical pharmacy services including medication reviews, drug interaction analysis, and medical management consultations for residents.
Collaborate with healthcare providers to develop and modify treatment plans, including pain management, immunizations, and pediatrics as needed.
Compounds medications using standard formulas and processes such as weighing, measuring and mixing ingredients.
Utilize pharmacy software, EMR (Electronic Medical Records), and EHR (Electronic Health Records) systems to document patient care activities accurately.
Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times.
Support pharmacy technician staff by providing guidance on medication preparation, inventory control, and medication administration procedures
Engage in patient care activities such as immunizations, medication administration education, and acute care interventions within the LTC environment
