What are the responsibilities and job description for the Health Ally (Patient Advocate / Care Navigator) position at StreamMD?
As a Health Ally (HA), you will serve as a bridge between patients, the community, and the broader healthcare ecosystem. This is a non-clinical role focused on understanding community health needs at a deeply human level.
You will visit select partner medical practices to engage directly with patients in their waiting rooms. Your mission is to listen to their stories, help them identify barriers to care (such as financial or access challenges), and ensure they feel supported and informed. Additionally, when a patient aligns with our primary care approach, you will introduce them to our membership-based primary care model at Wellspring Rhinebeck.
Key Responsibilities
- Patient Engagement: Conduct warm, brief, and empathetic 1-to-3 question conversations with patients in partner clinic waiting areas.
- Healthcare Navigation: Help patients understand their health options and identify overall healthcare system barriers (e.g., financial, insurance, or access challenges).
- Insight Gathering: Collect qualitative insights from patients regarding their health challenges (unrelated to the specific practice itself) to help improve regional quality of care.
- Practice Collaboration: Coordinate smoothly with partner clinic Office Managers and receptionists to ensure your presence reduces administrative burdens and enhances patient satisfaction.
- Practice Growth & Education: Explain Wellspring Rhinebeck’s core model and philosophy of primary care to interested patients, providing them with seamless pathways to sign up for memberships if they choose.

Location: Must be able to reliably commute or relocate to the Rhinebeck, NY 12572 area. Plus if local to this area.
Pay: $17.00 - $21.00 per hour
