Job Summary
Strawberry Fields, Inc. is seeking a Blended Case Manager (BCM) with experience working with children and adolescents to support individuals with serious and persistent mental illness. This position will involve working with a caseload that is 60% children/adolescent and 40% adult consumers. As a BCM, you will assess consumer needs, create tailored service plans, and provide ongoing support to ensure those needs are effectively met. You will work closely with individuals and families, coordinating resources and services to ensure optimal outcomes.
Vehicle Required: A privately maintained vehicle is necessary for daily client visits within the Centre County area. Candidates will possess a valid PA driver’s license with 3 years of driving experience and excellent driving history.
Job Responsibilities
- Support children, adolescents, and adults with serious and persistent mental illness by connecting them to appropriate resources
- Conduct assessments based on consumers' strengths and needs, and develop service plans every six months or as needed
- Monitor and evaluate the effectiveness of community resources and services
- Assist consumers in attending psychiatric, medical, and community-based services
- Participate in periodic on-call shifts (approximately 3 days per 27-day cycle)
- Maintain a caseload with 60% children/adolescent consumers and 40% adult consumers
- Conduct outreach efforts to connect consumers with additional resources and support
- Collaborate with mental health professionals, families, and other community stakeholders
- Maintain strict confidentiality and professional conduct at all times
- Regular attendance at meetings and required training
- Perform any other duties as assigned by the supervisor
Minimum Qualifications
- Required:
- Bachelor’s degree in a human services-related field or 12 college credits in human services with two years of related experience
- Privately maintained vehicle with a good driving record
- Criminal background check, personal, and professional references
- Strong communication skills to effectively interact with consumers, staff, families, and the community
- Possess the physical stamina and emotional stability to handle medical and psychiatric emergencies
- Preferred:
- One-year commitment to the position
- Experience working with children and adolescents
