Education Qualifications
- Bachelor's Degree Bachelor of Science in Pharmacy from an accredited school of pharmacy. Required or
- Doctorate PharmD from an accredited school of pharmacy. Required
- 3 years Relevant acute care pharmacy experience or has successfully completed a Post Graduate-Year 1 accredited pharmacy practice residency. Required
- Successful completion of a Post Graduate Year 2 accredited pharmacy practice residency program. Preferred
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
- Ability to apply advanced pharmaceutical mathematical concepts including but not limited to the following: pharmacokinetic calculations, Child Pugh score, Cockcroft- Gault calculations, and other pertinent calculations based on patient population.
- Ability to define problems collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

- As a central or decentralized pharmacist, review, interpret and verify provider medication orders making any recommendations and clinical interventions as necessary based on patient specific condition, disease state, age and other clinical factors. In a decentralized role will take responsibility for assigned area of coverage including attending multidisciplinary rounds, patient clinical review, and all operational aspects related to order verification and medication distribution.
- Manage medication use including the compounding, preparation, selection, dispensing, storage and stocking of medications for the entire organization. In a decentralized role will be responsible for pharmacy operations and patient care in surgery, Infusion Center, Cancer Center and the Emergency Department.
- Design, implement, and lead pharmacy initiatives in alignment with the department and organization mission, vision and structure to include finance, clinical, safety, automation, population health, and performance improvement. Work with other pharmacy leaders to collect, organize and effectively use date from initiatives to drive change and improvements.
- Address and complete provider consults proficiently, provide drug information to healthcare professionals and education to patients.
- Educate and train pharmacy personnel, healthcare professionals and serve as a preceptor for pharmacy students and pharmacy residents.
- Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
