Behavioral Health Coordinator/Provider (K-5)

Job Summary:


The Behavioral Health  Coordinator/Provider is responsible for ensuring that students with disabilities or medical/behavioral health needs in grades K-5 receive the necessary accommodations and services as outlined in their 504 Plans and Plans of Care (PoC). This role works collaboratively with school staff, families, and other educational professionals to ensure compliance with Section 504 of the Rehabilitation Act, state education codes, and best practices for meeting students’  medical/behavioral health and safety needs in the school environment. The coordinator will also monitor the effectiveness of accommodations and services, conduct ongoing evaluations, and ensure the educational success and well-being of students through direct individual or group intervention and coordination of services.  

 

Key Responsibilities:

  1. 504 Plan Coordination:
    • Coordinate and facilitate 504 Plan meetings for students, ensuring that all stakeholders (parents, teachers, administrators, and service providers) are involved.
    • Develop, implement, and monitor 504 Plans for students in grades K-5.
    • Ensure that 504 Plans are compliant and aligned with the individual needs of each student.
    • Provide training and resources for school staff on 504 Plan accommodations and modifications.
  2. Caring 4 Students Implementation:
    • Participate in ongoing school team meetings (Student Study Team (SST) or PIT Crew) to identify students that may require intensified behavioral health/medical interventions.
    • Collaborate with parents, school nurses, and healthcare providers to develop Plans of Care for students with behavioral health/medical needs that affect their school attendance, participation, or performance.
    • Ensure that each Plan of Care is developed and updated in accordance with state policy and identified student needs.
    • Document provision of services in alignment with the student’s Plan of Care and state policy.
  3. Collaboration with Stakeholders:
    • Act as a liaison between parents, teachers, counselors, and other school personnel to support the academic, social, and emotional success of students with 504 Plans or Plans of Care.
    • Provide families with information and resources related to 504 accommodations and Plan of Care services.
    • Conduct regular communication with families to review student progress and discuss any necessary changes to the 504 Plan or Plan of Care.
    • Serve as a resource to the school community for disability-related questions and concerns.
  4. Compliance and Documentation:
    • Ensure compliance with all legal and regulatory requirements related to 504 Plans and educational services for students.
    • Maintain accurate, timely, and confidential records of 504 Plans, Plans of Care, meeting notes, and communication with families and staff.
    • Prepare and submit required documentation for compliance reviews and audits, as needed.
    • Ensure timely updates and reviews of 504 Plans and Plans of Care, according to district/state/federal policies.
  5. Training and Professional Development:
    • Provide professional development for staff on topics such as Section 504, disability awareness, trauma informed practices, legal requirements, and best practices for supporting students with behavioral health needs in the school environment.
    • Stay informed on updates to relevant laws, regulations, and best practices to ensure the school remains compliant with all behavioral health and safety requirements through participation in relevant professional development opportunities and/or communities of practice. 
  6. Advocacy and Support:
    • Advocate for students with behavioral health and/or safety related needs within the school setting.
    • Work with educators and families to identify barriers to student success and recommend appropriate solutions.
    • Provide social/emotional/behavioral/safety related and/or academic support to students as needed to promote their success in the school environment  This may include providing immediate crisis intervention and de-escalation based on student and staff needs.  

Qualifications:

  • Education and Experience:
    • Licensed Clinical Psychologist, School Psychologist, Master’s level Social Worker, Board-certified Behavior Analyst, or Licensed Professional Counselor. 
    •  Must meet the State of Michigan licensing requirements 
    • Experience working with students with disabilities, medical conditions, or behavioral health needs, ideally in an elementary school setting.
    • Previous experience in developing and coordinating 504 Plans and/or school-based intervention plans is preferred.
  • Skills:
    • Strong organizational skills and attention to detail.
    • Excellent written and verbal communication skills.
    • Ability to collaborate effectively with diverse groups, including families, teachers, administrators, and healthcare providers.
    • Problem-solving skills and the ability to develop creative solutions for students’ needs.
    • Ability to maintain confidentiality and handle sensitive information.

COMPENSATION:  In accordance with the master agreement between Comstock Park Educational Employees Association and the Comstock Park Board of Education.

 

The final candidate for this position will be required (at their cost) to furnish the district with a current Michigan State Police and FBI criminal records LiveScan check prior to our recommendation to hire.
