What You Will Do as a Hospice Clinical Manager RN

As the Hospice Clinical Manager, you will be responsible for the day-to-day operational oversight and management of the hospice interdisciplinary team. You will be responsible for clinical oversight, implementation and ongoing assessment of the hospice plan of care, patient advocacy, monitoring patient/family satisfaction, data collection, reporting, quality initiatives, compliance, and ongoing education of the hospice team. Some of your primary responsibilities include:

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Provide direction, coordination, and evaluation of hospice care and services.
  • Participate in the orientation and retention of qualified hospice staff.
  • Assist in ensuring quality care consistent with hospice policies, Federal and State regulations, and accreditation standards.
  • Provide annual employee performance evaluations.
  • Assist in monitoring and ensuring patient, family, and referral satisfaction with hospice services. Address trends or problems with satisfaction.
  • Participate in the QAPI program specific to the needs of the hospice and ongoing collection of data to be utilized within the QAPI program that reflects best practice patterns, personnel performance, and patient outcomes.
  • Initiate therapy program and instruct other personnel and/or family/caregiver members in certain phases of therapy with which they may work with a patient, as well as instructing them as to the goals of the therapy program for the patient by participating in case conferences.
  • Participate in the development of the branch budget, monitor the budget, and provide justification for variance.
  • Run reports to support agency operations in relation to finance, billing, documentation, and patient outstanding items.
  • Support and adhere to the fiscal success of the agency in compliance with Board directives.
  • Working knowledge of hospice policies, Federal and State regulations, and Accreditation Standards and the ability to interpret for staff.
  • Demonstrate the ability to continuously prepare staff for successful regulatory and standards surveys.
  • Ability to hold staff accountable for compliance with regulations, policies and standards.
  • Recognize excellence and the need for improvement when monitoring clinical records and provide appropriate feedback.
  • Participate in training and verification of competencies of hospice staff.
  • Ability to ensure hospice care and services are provided according to the plan of care.
  • Ensure appropriate staffing. Provides relief and periodic patient coverage, if needed.
  • Maintain skilled nursing competency.
  • Demonstrate the ability and skills to assume the role of the Director during temporary absences and/or vacancies.
  • Ability to assist with budgeting, monitoring expenditures and provide sound business practices.
  • Ability to hold staff accountable for informing patients and their families regarding the costs of hospice care and services.
  • Provide supervisory visits to provide coaching and feedback.
  • Complete annual education and certification requirements as applicable.
  • Maintain patient, family and employee confidentiality.
  • Promote teamwork, accountability and positive attitudes.
  • Contribute to team effort by accomplishing related results, as needed.
  • Adheres to the Organization's documentation and care procedures and standards of personal and professional conduct

Education, Licensure & Certification Requirements

  • Active RN licensure
  • Minimum of three (3) years of hospice or home care experience
  • Minimum of one (1) year in a clinical management role
