Position Summary: The Care Consultant works with the highly complex patient, assessing the patient and their environment and then develops individualized care plans that impact health goals and reduces highly complex social needs. This position assists along any point of the care continuum where care is fractured, or where a transitional need is identified by: Providing an in-person assessment of the patient and their environment to identify barriers, with intensive, short-term follow-up either in person or telephonically. Utilizes motivational interviewing, care coaching and advanced engagement models to assess the patient's level of activation and to identify personal goals to align to the medical plan of care Addressing urgent high-risk and rising risk needs for harm-reduction and mitigation Connecting the dots between caregivers, community members, plan, and health care team members for clarity in the plan of care The successful Care Consultant has a high engagement rate, appropriate level of care planning, and measurable improvement in unplanned and avoidable utilization. The Care Consultant works with Complex, High-Risk patients, most often in a transitional space. The Care Consultant meets the patient where they are at -including in their home - assessing both them and their environment, to collaborate on a plan of care that puts the patient at the center. The Care Consultant practices the Quadruple Aim of reducing unnecessary costs and services to drive efficient quality outcomes while addressing patient and team satisfaction. The Care Consultant works as an intensive care manager across the care continuum that meets the patient in their home and works with both organizational and community-based partners. Travel will be required in the greater Stillwater/Western WI area. Work Schedule: Monday through Friday 8:00am-5:00, varying hours Required Qualifications: We are open to hiring either a Registered Nurse or LICSW Registered Nurse Minimum Qualifications Current Minnesota RN license WI license must be obtained within 6 months BS, BA or AD in nursing from a college or university. Minimum of 3 years of experience within an adult acute care setting, hospital, home health setting or a managed care company, or service of vulnerable adults. LICSW Minimum Qualifications: Master's degree in social work Minimum of 2 years healthcare experience required. Active, unrestricted SW license in the state of MN required. WI license to be obtained within 6 months Regional travel required: valid MN driver's license and reliable vehicle. This position provides psychotherapy to patients and requires Park Nicollet to comply with the Minnesota Statute MS603 (formerly 148A). Preferred Qualifications: BS or BA preferred. Minimum of 2 years Care Management experience preferred.
