Registered Nurse, Hospice Care Case Manager

This is a full-time, on-site position for a Registered Nurse, Hospice Care Case Manager, based in Warsaw, IN. The role involves managing and coordinating patient care plans, providing nursing care, and offering support to ensure the comfort and well-being of hospice patients. As a case manager, you will collaborate with an interdisciplinary team to deliver personalized medical care, guide families through the process, and provide critical care and emotional support. The role is essential to delivering exceptional and compassionate end-of-life care.

• Strong skills in Nursing and Patient Care to assess, plan, and implement patient care effectively.
• Expertise in Medicine and Critical Care Medicine to provide high-quality hospice care and address complex medical needs.
• Proficiency in Nursing Management to oversee and coordinate care plans with a multidisciplinary team.
• Excellent communication and interpersonal skills to support patient needs and provide guidance to families.
• Valid Registered Nurse (RN) license in the state of Indiana and any required certifications for hospice care.
• Experience in a hospice or palliative care setting is preferred but not mandatory.
• Compassionate approach and commitment to delivering patient-centered end-of-life care.
