Responsibilities
- Conduct comprehensive patient assessments to determine eligibility and develop individualized hospice care plans aligned with patient needs and preferences.
- Manage clinical casework by coordinating services, monitoring symptom management, and adjusting care plans as necessary to optimize comfort and quality of life.
- Collaborate effectively with healthcare professionals across disciplines—including physicians, social workers, chaplains, and therapists—to ensure holistic patient support.
- Provide education to patients and families about hospice services, medication management, symptom control, and advance directives to empower informed decision-making.
- Utilize electronic health records (EHR) systems for accurate documentation of patient progress, medical records, discharge planning, and compliance with HIPAA regulations.
- Oversee discharge planning and transition processes from hospital or inpatient settings to home or hospice facilities, ensuring smooth continuity of care.
- Participate in ongoing quality improvement initiatives related to hospice care standards, infection control policies, and health regulation compliance.
Qualifications
- Valid nursing license with current registration
- Experience with hospice, home health or outpatient clinics
- Strong knowledge of hospice, medical terminology, health regulation policies (including HIPAA), and electronic health records (EHR/EHR management).
- Proven ability to perform comprehensive patient assessments and manage complex clinical cases
- Excellent communication skills for client interaction and interdisciplinary collaboration; adept at managing patient service needs with compassion and professionalism.
- Familiarity with managed care processes, utilization management, discharge planning, diagnostic evaluation, and case management protocols.
